Before applying for an authorization you must contact the Benefit Department to confirm the members coverage and benefit for each item along with the corresponding diagnosis code. Fidelis has, as of July 1, 2011, promulgated new procedures for securing authorizations and paying claims. These new rules have previously been faxed to all offices. If you do not have the fax please view our website at www.integrah.com, click on “Provider”, then “Billing”. PLEASE PAY PARTICULAR ATTENTION TO THE SECTION ON SHOES. THAT SECTION IDENTIFIES ALL CODES REQUIRING THE PRESCRIBING PHYSICIAN TO SECURE THE REQUISITE AUTHORIZATION.
